Seniors Exercise Program

At Community Therapy, we believe in helping older adults to live longer and better at home.

Our free Seniors Exercise Program is made possible by funding from the Hunter New England and Central Coast Primary Health Network (the PHN) through the Australian Government’s PHN Program.

The program entitles an older adult to:

  • 12 group exercise classes facilitated by an Exercise Physiologist
  • 3 individual home visits from an Exercise Physiologist or Physiotherapist

Group exercise classes are held at community locations across the Central Coast and Hunter.

Who is Eligible for the Program?

This program is designed for older adults living with chronic health conditions who are not currently receiving support through other funding streams. The eligibility criteria is:

  1. 65 years of age or older (55 years of age or older for First Nations)
  2. Living with at least one chronic health condition such as, but not limited to:
    a) Heart and lung conditions
    b) Diabetes
    c) Stroke and other neurological conditions
    d) Osteoarthritis and Osteoporosis

We are especially committed to reaching those who face greater barriers to healthcare access, including First Nations Australians, people with disabilities, and those from culturally and linguistically diverse backgrounds.

If you are already supported through programs such as HCP, CHSP, STRC, DSOA, or the NDIS, we have other exercise groups available in the community that may be a better fit. You can enquire about them here.

What to Expect from the Program

There are 4 stages within the 12-week program:

    • Initial Assessment: We begin with a 1-1 home visit by an Exercise Physiologist or Physiotherapist who will assess your current health and fitness using internationally recognised assessments, help you set meaningful goals and get ready for the group program.

 

    • Weekly Group Exercise Sessions: Participate in weekly 1 hour group exercise classes designed to improve strength, balance and mobility, all led by a qualified Exercise Physiologist.

 

    • Mid-Program Check-In: Midway through the program, your Exercise Physiologist or Physiotherapist will revisit your home to review your progress, address any challenges you may be facing, and discuss healthy ageing strategies.

 

    • Post-Program Review: At the end of the 12 weeks, your Exercise Physiologist or Physiotherapist will conduct a final assessment to measure your progress and provide guidance on maintaining a healthy lifestyle moving forward.
