Scoring the Waterlow Pressure Ulcer Risk Assessment
Pressure injury care and management is a vital tool in a clinician’s toolbox when working with clients across the lifespan within the community setting. The Waterlow Pressure Ulcer Risk Assessment is a popular screening tool used to assist in determining a client’s pressure risk.
This bite size training (7 minutes to be exact!) has been developed to provide additional details around the scoring system for the Waterlow Assessment, to allow it to be scored consistently across any team or organisation.
Disclaimer: This training reflects how Community Therapy utilises and scores the Waterlow Pressure Ulcer Risk Assessment.
The Waterlow Manual (2005) states explicitly that ‘…like all simplistic scoring systems it cannot scientifically predict with 100% precision the chance of a patient developing a pressure ulcer. What it can do is ensure that all patients are assessed objectively to the same system… the Waterlow card is another weapon in the armoury of nurses [or other Health Professionals], it is not a replacement for their knowledge and understanding.’
Some sections of the Waterlow Risk Assessment have a scoring range which Health Professionals are expected to interpret based on experience. We have provided additional guidance on how we interpret these scores as an organisation beyond what the traditional Waterlow Score Card provides.
