As an approved CHSP provider, our wonderful team of Physiotherapists, Occupational Therapists, Exercise Physiologists, Speech Pathologists and Dietitians deliver therapy and support to clients with CHSP Allied Health funding.
Commonwealth Home Support Programme (CHSP) is a funding scheme by the Australian Government designed to help older adults stay independent and active within their communities.
This scheme is an entry level funding program that provides access to a range of essential services, including allied health support, for people 65 years and older.
How to Refer to Us?
If you already have a CHSP code for allied health services, you can refer directly to Community Therapy via our referral form. We handle the My Aged Care portal for you, ensuring a smooth referral process.
Steps to Refer:
- Get your CHSP code: To access CHSP services, ensure you have a CHSP code specifically for allied health. You will receive this from My Aged Care after an aged care assessment.
- Contact us directly: With your CHSP code, reach out to us by phone or through our website. Our admin team will guide you through the referral process and ensure we have everything needed to start providing services.
If you don’t yet have a CHSP code, you can contact My Aged Care directly for eligibility and assessment.
What Can CHSP Clients Expect From Community Therapy?
Once we accept a referral, our wonderful administration team will call you to discuss our services and complete our pre visit questions. This ensures we understand your goals, how to best support you and how to provide services in a way that makes you comfortable.
An initial assessment with one of our experienced allied health professionals will then take place in the comfort of your home with anyone that you wish to be present being their as well.
This may be with one of our Physiotherapists, Occupational Therapists, Exercise Physiologists, Speech Pathologists or Dietitians. It will depend on what you have requested or what your clinical care need may be. Often, people have the support of several of our clinicians.
We then discuss with you the frequency of visits required to support you to achieve your goals.

Where do I find my CHSP code?
You will find your CHSP code(s) in your my support plan.
After an aged care asssessment by the regional assessment service or aged care assessment team you will be provided approval for different aged care services.
You will likely receive your my support plan via email.
You can also call My Aged Care if you have lost this information.

Who is eligible for the CHSP program?
To be eligible for Commonwealth Home Support Programme (CHSP) funding you need to be aged:
– 65 years or older (50 years or older for First Nations people); or
– 50 years of older (45 years or older for First Nations people), on a low income and homeless, or at a risk of being homeless.You will need to have an aged care assessment and this can be organised by contacted My Aged Care.

What is the difference between CHSP and Home Care Packages?
CHSP is focused on supporting older adults living with low level care needs whilst the Home Care Package scheme is focused on supporting people living with more complex care needs.
CHSP is not means-tested whereas Home Care Packages are.

Who can provide CHSP services?
Only registered CHSP providers can provide CHSP services.
Community Therapy is a registered CHSP provider for allied health services such as Occupational Therapy, Physiotherapy, Speech Pathology, Dietetics and Exercise Physiology.

Is CHSP being phased out?
The Department of Health and Aged Care have advised that CHSP will continue to at least 30 June 2027.
The intention is for CHSP to eventually merge with the Support at Home program.
The Support at Home program is combining the Home Care Package and Short Term Restorative Care Programs from 1 July 2025.
